Terms of Service of Artemis Smiles Orthodontics

These Terms of Service (“Terms”) govern your use of services provided by Artemis Smiles Orthodontics (“we,” “us,” or “our”), including our website, online scheduling, patient communications, and orthodontic care. By using our services, you agree to these Terms.
Services
Artemis Smiles Orthodontics provides orthodontic evaluation and treatment. Information on our website is for general informational purposes and is not a substitute for professional dental or medical advice.
Appointments and Communications
We contact you about your appointments and your account by phone and by email. We also send text messages, but only to patients who have separately opted in to receive them, as described in the “SMS/Text and Call Communications” section below. Giving us your phone number does not by itself sign you up for text messages.
Payment and Insurance
You are responsible for charges for services rendered, subject to any applicable insurance coverage. Details are provided at the time of treatment.
Privacy
Your information is handled in accordance with our Privacy Policy.
Changes to These Terms
We may update these Terms from time to time. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.
Governing Law
These Terms are governed by the laws of the State of Georgia, without regard to its conflict-of-law provisions.
SMS/Text and Call Communications
If you opt in, Artemis Smiles Orthodontics sends you appointment reminders, confirmations, scheduling and rescheduling notifications, treatment updates, insurance and billing notifications, and occasional practice announcements by text message (SMS) at the mobile number you provided, including messages sent using an automatic telephone dialing system. We do not send marketing or promotional text messages.
Opting in is voluntary and is always a separate, affirmative choice. Simply giving us your mobile number, booking an appointment, submitting a form, or accepting these Terms does not opt you in. Consent to receive text messages is not a condition of booking an appointment, of purchase, of creating an account, of completing any transaction, or of receiving treatment. If you decline, you receive exactly the same care and we contact you by phone and email instead.
We may also call you at the number you provide about your appointments and your account, whether or not you have opted in to text messages.
How you opt in
You may opt in in any of three ways:
- Online booking. Tick the text-message consent box — shown unchecked, and never pre-selected — beneath the contact fields on our online booking form. Your appointment is booked whether or not you tick it.
- New-patient intake forms. Answer Yes to the text-message consent question in the intake packet. There is no default answer, and both answers submit the form.
- Paper intake in the office. Sign the same consent language at the front desk.
Leaving the box unticked, or answering No, adds your mobile number to our do-not-text list, and we will not text you.
The consent language on our online booking form reads:

Consent is optional — it is not a condition of booking an appointment or of receiving treatment. Message and data rates may apply. Message frequency varies. Reply STOP to opt out, HELP for help.
The text-message consent question on our new-patient intake forms reads:

We store the wording you were shown, together with a timestamp, as the record of your consent.

- Carriers are not liable for delayed or undelivered messages.
Privacy
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging originator opt-in data and consent are not shared with any third parties.
